Small Fiber Neuropathy (SFN)
Indication
Small fiber neuropathy and suspicion of inflammatory sensory neuropathy.
Clinic
IgM-antibodies against TS-HDS are reported in a subgroup of patients with small fiber neuropathy (SFN) with unknown underlying cause. These antibodies activate complement and immune therapy may be effective, clinical trials are ongoing. TS-HDS IgM seems to be more frequent and specific for SFN than IgG-antibodies against fibroblast growth factor receptor 3 (FGFR3). Antibodies against FGFR3 have been reported in patients with autoimmune diseases and concomitant sensory neuropathy. Plexin D1 antibodies have been detected in patients with burning pain and thermal hyperalgesia. The symptoms often respond to immunotherapies.

Fintrådsneuropati (SFN)

Fintrådsneuropati och misstanke om inflammatoriska sensorisk neuropati
Klinik
IgM-antikroppar mot TS-HDS har rapporterats förekomma i en subgrupp av patienter med fintrådsneuropati utan annan känd etiologi. Antikropparna är komplementaktiverande och immunosuppressiv behandling kan ha positiv effekt, kliniska studier pågår. TS-HDS IgM förefaller vara vanligare för fintrådsneuropati än IgG-antikroppar mot fibroblast growth factor receptor 3 (FGFR3). Antikroppar mot FGFR3 har rapporterats hos patienter med autoimmun systemsjukdom och samtidig sensorisk neuropati. Plexin D1-antikroppar kan påvisas hos patienter med neuropatisk smärta. Brännande smärta och värmeöverkänslighet typiskt och symtomen svarar ofta på immunoterapi.
